Answer all questions during the nomination process. No other attachments (such as letters of recommendation) will be accepted.

If your nominee is chosen, you will be asked to provide the nominee’s signature and photo (headshot, JPG/TIF format, 300 DPI minimum)

Alternate Method of Nomination Submission: If you can not submit your nomination online and need to e-mail it to us, please use this link Nominations Packet to download the nomination packet. When prompted – SAVE the file to your DESKTOP and give it a name you will easily remember. Complete the nomination form (it is a Microsoft Word document) and email it to us at NPD@afpwcc.org AND admin@afpwcc.org.
